INFORMED AGREEMENT TO THE PROCESSING OF PERSONAL DATA
By checking the box during browsing of website https://flashua.org «I consent to the
collection and processing of my personal data under the conditions and in the manner
specified in the Informed Agreement to the Processing of Personal Data». I, the subject of
personal data, provide my voluntary, unambiguous, specific and informed consent to the
processing of my personal data for the Foreign Recruitment Center as the owner of
personal data in accordance with the conditions below:
1) Composition and content of my personal data that may be processed: surname, first
name, patronymic, relationship to military service, telephone number, email address, unit
in which the person is serving (or plans to serve) military service, age and date of birth,
residential address, position in the military unit and other data contained in the
questionnaire that the person fills out on the website.
2) The purpose of processing my personal data:
- ensuring the use of https://flashua.org;
- improving the operation of https:/flashua.org;
- selection for me (at my request) of a vacancy in a military unit of the Defense Forces of
Ukraine; - checking information about me in available databases, the Internet, etc. to identify cases
of criminal liability, participation in illegal or other activities that contradict the interests of
Ukraine; - checking information about me in available databases, the Internet, etc. to identify cases
of criminal liability, participation in illegal or other activities that contradict the interests of
Ukraine; - assistance to me (at my request) with resolving issues related to recruiting to the
Defense Forces of Ukraine.
3) The range of processing of my personal data may include, including any action or set of
actions, such as collection, registration, accumulation, storage, adaptation, modification,
renewal, use, dissemination (transfer), depersonalization, destruction of my relevant
personal data. At the same time, the Owner is granted permission to transfer my personal
data, including, but not limited to, granting partial or full rights to process personal data, to
any personal data managers and third parties, within the range of the above-mentioned
processing purpose and subject to compliance with the requirements of applicable
legislation in the field of personal data protection.
This Informed Agreement is complete and applies to all cases of transfer of my personal
data to any personal data managers and third parties in Ukraine and abroad, therefore the
Owner does not need to obtain my additional consent for each such transfer. The owner
does not need to additionally notify me of each transfer of my personal data to any
personal data managers and third parties. The party to which my personal data is
transferred shall first take all measures to ensure the requirements of applicable legislation
in the field of personal data protection.
The personal data manager may process personal data only for the purpose and to the
extent specified in the contract concluded with such personal data manager in writing, and
may not exceed the purpose and scope of personal data processing specified in this
Informed Consent.
4) The manager of my personal data may be any natural and/or legal person whose
participation is necessary to achieve the purpose of personal data processing defined
above.
5) The mechanism and means of processing my personal data include, among other
things, processing using information (automated) systems.
6) The term of processing my personal data is limited to the period determined by the
purpose of processing personal data, as defined in this Informed Consent, unless I
withdraw this Informed Consent earlier.
In addition to the above, by checking the box when using https://flashua.org; in the item
“I consent to the collection and processing of my personal data under the conditions and in
the manner specified in the Informed Consent to the Processing of Personal Data” I also
confirm that I have been informed, explained and understood all of the following: - I am informed that this Informed Consent may be revoked by me at any time by
sending a letter to the email address info@flashua.org. The withdrawal of this
Informed Consent shall take effect from the date on which the Owner confirms my
identity and receives my corresponding expression of will. At the same time, I
understand and agree that in the event of withdrawal of the Informed Consent, from
the moment such withdrawal takes effect, the purpose of processing my personal
data specified in this Informed Consent can no longer be achieved.

personal data database of the Owner, as well as my rights as a personal data
subject, defined in Part 2 of Article 8 of the Law of Ukraine “On Personal Data
Protection”, namely:
1) to know about the sources of collection, location of my personal data, purpose of
their processing, location or place of residence (stay) of the Owner or manager
of personal data or to give the appropriate instruction to obtain this information
to authorized persons, except for cases established by law;
2) to receive information about the conditions for providing access to my personal
data, in particular information about third parties to whom my personal data is
transferred;
3) To have access to my personal data
4) To receive a response on whether my personal data is being processed, as well
as receive the content of such personal data, no later than thirty calendar days
from the date of receipt of the request, except in cases provided for by law;
5) To submit a reasoned request to the Owner objecting to the processing of my
personal data;
6) To submit a reasoned request to change or delete my personal data to any
Owner and manager of personal data if this data is processed illegally or is
inaccurate;
7) to protect my personal data from unlawful processing and accidental loss,
destruction, damage due to intentional concealment, failure to provide or
untimely provision, as well as to protect against the provision of information that
is unreliable or defamatory of the honor, dignity and business reputation of an
individual;
8) to file a complaint about the processing of my personal data with the
Commissioner for Human Rights of the Supreme Council of Ukraine or to the
court;
9) To apply legal remedies in case of violation of legislation in the field of personal
data protection;
10) To make reservations regarding the restriction of the right to process my
personal data when providing consent to the processing of my personal data;
11)To withdraw consent to the processing of my personal data;
12)To know the mechanism of automatic processing of my personal data;
13)To protect against automated decision-making that has legal consequences for
me.
In addition to the above, by checking the box when using https://flashua.org; in the item
“I consent to the collection and processing of my personal data under the conditions and in
the manner specified in the Informed Consent to the Processing of Personal Data” I also
confirm all of the following: - I confirm that the personal data I have provided relates to me personally, and is
provided voluntarily and in an informed manner.
I confirm that at the time of checking the box “I consent to the collection and
processing of my personal data under the conditions and in the manner specified in
the Informed Consent to the Processing of Personal Data” while using https://
flashua.org, I have been duly informed about the owner of my personal data, the
composition and content of the collected personal data, my rights as a subject of
personal data, defined by the Law of Ukraine “On Personal Data Protection”, the
purpose of collecting my personal data and the persons to whom my personal data
may be transferred.
